New Patients

Approximately seven days prior to an appointment with us, you will receive a link in your appointment reminder (via email and/or text) to complete your intake forms. To streamline your check-in process, please complete these forms ahead of time. You can either bring completed forms with you, text them to our practice, or email them to us ahead of your appointment at info@apexskincenter.com.

We encourage you to sign up for our patient portal where you can access your visit notes and view results easily online. We also provide the option of paying your bill online.

At Apex Skin Center, we seek to deliver exceptional patient care. We are here for you and your skin. Below are some of our practice’s policies that allow us to provide consistently high-quality care and live up to our core values and mission. Thank you for reviewing these policies.

Our providers see patients by appointment only.  We are not able to accommodate walk-in appointments. If you have an urgent dermatology need, please contact our office by phone or text and we will work with you to find the best possible appointment.

Arriving late to appointments impacts your provider’s and other patient’s schedules. We pride ourselves in keeping wait times to a minimum and timely arrivals ensure our patients are seen on time. If you are late, and the provider does not have an opening in their schedule, you will need to be rescheduled. If you know you are going to be late, please call us to see if you will still be able to be seen. Please plan to arrive 10 minutes prior to your appointment time if you are an established patient or 20 minutes prior to your appointment time if you are a new patient.

Any patient who is more than 15 minutes late for their appointment or has not completed their paperwork, will need to be rescheduled.

Medical Visits

When you schedule an appointment with our practice, we set aside enough time to provide you with the highest quality care. Should you need to cancel or reschedule your appointment, please contact our office as soon as possible and no later than 24 hours prior to your scheduled appointment. This gives us time to schedule other patients who are waiting for an appointment.  We realize there are times when emergencies arise that may cause you to miss an appointment, but excessive no-shows or same day cancellations are disruptive to our practice and other patients.  Below is our No Show/Appointment Cancellation policy.

No-shows for medical appointments may be charged a no-show rate of $50.00. This charge is not covered by your insurance plan.

Any established patient who fails to show or does not cancel or reschedule within 24-hours of their appointment three times within a twelve-month period will be discharged from our practice.

Any new patient who fails to show for their initial visit two times will not be rescheduled.

Surgical and Cosmetic Visits

Unless cancelled at least 24 hours in advance, missed cosmetic and surgical appointments will be charged a $200. This charge is not covered by your insurance plan.
